For counseling and art therapy clients, the following forms will help us get to know each other better before we begin.  You may download each one by clicking the links below: 

1.  Please read and save the "Introduction to Therapy Services" 

2. Please print, complete, and bring the following documents to our first meeting:

a)  "Adult Intake" or "Child/Teen Intake"

b)  "Consent for Therapy"

Payment

I accept all major credit cards, as well as cash/checks, through my office.  If you would prefer to pay on-line, please let me know.  I can e-mail you an invoice with a link for easy on-line payment.
